Be cautious when the provider offers only corporate documents in response to a facility-specific question. Challenge blanket payment promises
Ask the provider to explain the employer-funded scope, exclusions and individual payer-confirmation process. Statements suggesting that every admission or service is covered should be made precise and reviewed before being shared with employees.
